Items you may need ahead of time before starting your application:
- Copy of your IRS SS-4 Form
- Proof of Ownership or Fully Executed Lease
- For New Buildouts, Plumbing Plan Showing Fixtures Units
- Food/Drink Service
- Copy of the Menu
- Floor Plans with seat count & grease trap information.
Applications will be reviewed, and a response will be provided within 2-3 business days. Additional documentation may be requested based on the typical of commercial establishment, please make sure the application includes contact information.
Your business may require a Backflow Inspection to be completed by one of the Approved Backflow Contractors upon service transfer. If you have questions on whether your location is compliant, contact our Backflow Prevention Administrator at 904-213-2466 or
backflowadmin@clayutility.orgA cross-connection inspection will be required if your business has reclaimed water for irrigation. The inspection last approximately ten minutes and will require an authorized business agent (over 18) with access to the irrigation control panel to be present.
An interconnection inspection may be required if your new business occupies a unit or suite in a building with multiple units. This inspection verifies that the unit or suite is connected to the correct meter.
